Complete intake form within 10 hours of booking first service.

office / access:

* bring cellphone to call/text number in your appointment confirmation email upon arrival

* there is often a golden retriever dog in our office

* essential oils are lightly diffused in our office

* stairs are required to reach 2nd floor office

* street parking only-- free!-- usually found within 3 blocks

insurance reimbursement:

* if you intend to bill your insurance for reimbursement,

  tell us at beginning of appointment and bring your doctor's prescription

frequency:

* the body loves a rhythm!  (schedule appointments in a predictable frequency)

* we recommend weekly sessions to address an issue, or

 sessions once or twice a month for health maintenance

preparation:

* bring your own water bottle for post-session hydration

* phone sessions: minimize distractions and fully charge phone

* wear stretchy and comfy clothes

some policies:

* if you must cancel, please do so at least 24 hours before appointment time, 

  to receive full prepaid credit

* booking an appointment means that Square will charge

     your credit card for the full appointment amount

* once your credit card is charged, we do not offer refunds-- 

     only a full prepaid credit is possible as long as 

  you cancel 24 hours before appointment

* if you are late, we'll wait 15 minutes-- after that we'll leave the office

* there are no credits or reschedules if you are more than 15 minutes late

* if you are sick or may be coming down with something,

      we cannot see you in the office

* if you notice symptoms within 24 hours before your appointment, 

      email us as soon as possible and we can shift to

  phone appointment or work something out
