Accident Visit: We bill 1st-party auto accidents and 3rd-party accidents only with he use of an Attorney.
You MUST have the information below before the 1st Auto Accident Visit:
1) Auto Insurance name, Claim Number, Contact name and phone number of the Insurance agent handling your PIP claim, attorney, and a fax number to which to fax the bill.
2) You will need a prescription from a GP, chiropractor, naturopathic, or medical doctor. You must have the prescription with the diagnosis code by the first appointment. Ask your doctor's office to fax the prescription to 425-777-2100.
3) **Valley and Eastside do not bill regular medical insurance. However, many insurance companies do reimburse you for massages. We will provide you with a receipt that you can submit to your insurance company for reimbursement. It is your responsibility to determine what your insurance covers and what it reimburses you for.
You will need a prescription from a Doctor or an NP. PA or Chiropractor with a billing diagnosis code or an ICD-10-CM code from them.
