Home Care service provision primarily for Waukesha/Jefferson Counties. Responsible for providing high level care as both physical therapist and case manager to patients in AL/IL facilities, SNFs and patient’s own homes. Primarily working with geriatric population with CHF, COPD, Parkinson’s Disease, Multiple Sclerosis, and high falls risk, including palliative and hospice. INR and LVAD trained.
