Home Care service provision primarily for Waukesha/Jefferson Counties. Responsible for providing high level care as both physical therapist and case manager to patients in AL/IL facilities, SNFs and patient’s own homes. Primarily working with geriatric population with CHF, COPD, Parkinson’s Disease, Multiple Sclerosis, and high falls risk, including palliative and hospice. INR and LVAD trained.

Providing inpatient rehabilitation services to short and long term care SNF residents and outpatients. Primarily working with sub-acute care patients with varying conditions/needs. Acted as Therapy Program Manager as required.
